LTC to SNF

Hi MDS team.

Is it true that any time there was a level of care change, it was considered significant. If a resident previous status was private pay, and when returned from the hospital, was admitted under her Medicare a benefit, which would warrant a significant change/5day assessment

Featured Replies

Not necessarily. The criteria for SCSA must still be satisfied.

The team has 14 days to decide whether the changes are permanent or prolonged and another 14 days to do the SCSA once determined. Doing the SCSA prematurely will compel you to do another SCSA for improvement once the resident convalesces.
