Need some info on LTC Careplans..Just started working in LTC. At our facility one RN careplans on 112 residents. We have an old DOS pakage for computerized care plans but it seems more trouble than its worth...was wondering if anybody is using an efficient comuterized method for initiating and updating careplans.

If your not happy with your software you have now, when doing MDS and careplanning, then maybe you need to voice your concerns with management. We use computerized care planning, it starts off as a canned care plan that is modified to meet the needs of that specific resident. I also get discouraged at the lack interests by others, but there is growing interest in the facility I work at. The nurses aids and also housekeepers attend our weekly care plan meetings. This is basically manditory at my facility. This is very helpful also. On occasion a floor charge nurse attends if needed. Granted it is usually a brief interview, but also gets them involved in the care planning process.

I have worked in Many LTC facilities. I remember when the MDS's were initiated. Some places just plug in their own data, not always factual but they made everything jive. I overheard an MDS cordinator tell an LPN to just copy what the day nurse wrote. Other facilities actually wanted the information to be acurate and the only place to get that info was from the nursing staff. Most places that I worked at had initial careplans that the admitting nurse initiated. all the other care plans are to be added to when ever a nurse feels something should be on there. Some places still have the nurses do the paper MDS and then the cordinator plugs it into the computer. of coorifice she gets the credit for bringing in the dollars. HA HA

I was taught that Nursing Care Plans are the key to professionalism. I write most of the careplans in my facility (160-200 beds) and I love it- I beleive in careplans so that the left hand knows what the right hand is doing. I make sure that everyone reads them by keeping them breif, to the point and in legible handwriting. I institued a change by attaching them to the nurses notes and even encourage nurses to refer to them in her notes (e.g. "care given according to care plan"). I am frequently complimented on them especially by surveyers.

Bottom line is if you throw in the towel and treat them as unimportant, irrelevant and a waste of time, you are throwing out an important aspect of our professionalism
