LPN Letter of Recommendation. HELP!

The physician I work closely with is leaving our clinic soon, 7 days! We work in a very small practice where he had often seen me administer injections, draw blood, process blood and urine work and dress wounds. Even assisted with the removal of sutures several times.He has told me that if I write the letter of recommendation he will sign it, he doesn't write English very well. I really could use some help because, I sound like an idiot compared to what a physician would actually write! I've tried almost 10 times now and end up trashing it!

Conduct a Google search using the term 'professional recommendation letter.' Write the letter based on a template you like. Change some of the words around.

I agree with TheCommuter. Google some letters of recommendations for nurses. I'm sure there is something you can use. Of course try to change it around and make it in your own words.

Usually, letters of recommendation are started like this:

If your facility has a official letterhead, try to have the letter on that.

This is how I would write one:

To Whom It May Concern:

I am writing this letter to offer my highest recommendation for Michelle for consideration as a staff nurse. (Or you can say "concerning x position")

- Next you need to write how this person that's writing the letter knows you.

Ex: I am the RN supervisor at xxxx. Or " I have worked alongside Michelle as.." Or "I was Michelle's lecture instructor for Nursing 205".

Then you can write qualities about yourself.

Ex: Michelle consistently delivered the highest quality patient care. She is enthusiastic, focused, displays great initiative. Pretty much write stuff to see yourself.

Lastly, you should close your letter again stating that they recommend you.

So example: I recommend her for employment without reservation. For any further information, please feel free to contact me.

Sincerely,

Boss

Title (In your case it would be Physician)

Or you can write something like: It is with great pleasure that I recommend Michelle for a position as staff nurse. If I can be of further assistance, please do no hesitate to call me at xxx-xxx-xxxx.
