I have my doctor's appt tomorrow and I just wanted to ask about the physical form the doctor is supposed to sign. I know they are supposed to sign it and stamp it .Do they sign it at the signature portion and the stamps only go in the front and back.I just wanted to confirm so I don't make a mistake LOL

Technically the doctors do not complete any forms, it is supposed to go through Release of information department (ROI) however it is at the doctors discretion to complete it for you or send you to ROI, I believe ROI can take up to 10 days but you should contact them to be sure of processing times. If you have not completed labs the physical form cannot be completed until the results come back.

After your bloodwork is complete it will tell what vaccines you need due to not having immunity. I read in the information page that by the time the program starts, we'll need to have 2/3 of the shot series completed if we are not immune. Most vaccinations times are scheduled at least 30 days apart from the initial vaccination, so starting asap would be playing it safe. Hope this helps.

I had my physicals done today.For anyone who had their physicals done at kaiser ,how did you get your color blindness screening done.My kaiser don't do color blindness screening.And also can we print out our lab results and take it to the school health center to fill the physical form,sign it and stamp it if we are unable to do it through kaiser .Will the school also be able to sign it and stamp it

You can print out all your labs results from your online Kaiser account. Color blind screening is done by Kaiser optometry and when I last checked there were no screening appointments til February. So I had me color screen portion done by the college. From what I understand the college health center can complete your form for you as long as you take all the information from your physical. Also the eye screen with the health center was free.
