  2. Hi, I'm an LVN accepted into the nursing program. I am scheduled to test out of the first semester. The tests is basically to determine whether I will start at the 1st semester or 2nd. I was told either way I may start February 9th since they have continuous cohorts.
  3. I went to the school earlier today and purchased it, $7. Does anyone know if we're supposed to wear scrubs to boot champ?
  4. You can print out all your labs results from your online Kaiser account. Color blind screening is done by Kaiser optometry and when I last checked there were no screening appointments til February. So I had me color screen portion done by the college. From what I understand the college health center can complete your form for you as long as you take all the information from your physical. Also the eye screen with the health center was free.
  5. Technically the doctors do not complete any forms, it is supposed to go through Release of information department (ROI) however it is at the doctors discretion to complete it for you or send you to ROI, I believe ROI can take up to 10 days but you should contact them to be sure of processing times. If you have not completed labs the physical form cannot be completed until the results come back.
  6. If you can't get an in person appointment try to schedule with any doctor available or even a video or phone appointment. It takes a few days to get the results back after your labs are drawn. Can't wait for orientation!! ?
  7. I only had my $5 copayment, the copayment differs from patient to patient. I'm actually an LVN for Kaiser also.
  8. Is anyone doing the LVN to RN pathway? I just attend the zoom meeting for our additional requirements. Wondering if you received a study guide?
  9. Hi , I have Kaiser also, I got lucky .. my doctor completed my physical form for me. However, I couldn't get a color blindness screening with Kaiser optometry until February, so I was seen by the nurse from the health center and she screened me and reviewed my physical exam form to make sure it was correctly completed by Kaiser ...just call the health center and make at appointment. also in the zoom meeting they mentioned Kaiser and the physical forms , so I believe you can complete all your labs and physical at Kaiser and take your results to the health center along with your blank physical form and they will complete the form for you using results , print out, all your results, immunizations, and visit information from Kaiser and take to the health center at the day of your appointment. call the school health center to confirm. Physical exam and color blindness screening are free as a student . If you need labs then there is a charge hope this helps
