Looking for a good way to track my Pt.s

Hi,

I've been in HH for a few months. I'm trying to create a document for myself to track my patients visit frequencies/ when appts are neede/ next recert dates etc. to plan my week/month and not miss anyone and prepare for future recerts/ROCs etc. My company is all by laptop but I wanted something to print out so I can see/add my patients all to one document to get an accurate overview.

Has anyone created a document that works for them?

I used a calendar app specifically for my patients and nothing else. I was able to populate repeating appointments including reminders or ticklers to remember tasks that should be planned for. (ie: change foley, fill mediset, whatever works for you) I used first names and last initials in the app.

I went old school and bought a planner. It has space to write for a full day for each date where I list all my patients I will see for the day as well as a full calendar for each month so I can see it all laid out together. This is where I list recert dates so I can be able to count the window and plan what day to see the patient for the recert. I place my patients on the full month calendar based on their weekly frequency and then I pull them out and place them on the daily pages. It helps me to be able to see the big picture and I can also plan to see people who live in the same area on the same day. We have Ipads but I prefer my planner.

Any one work as a pay per visit RN for PCW services? I am not paid for office time so it's difficult to get a good system of organization on achedule? Maybe I should have started a new post but I am in a hurry and thought I would just ask to see if any oof you are able to access ct info and what the agency should be sharing with you?

Even staff who are paid per visit should have mechanism to get paid for administrative duties required by the employer.

This sounds like great advise. I'm starting a RN Case Manager job in HH in a couple weeks. Recommendations for best calendar app?

I use Outlook now but I used what ever calendar app was loaded on the device provided me by the employer. They are all a bit different but most allow you to create repeating events/appts and have some sort of "notes" where you can create HIPAA friendly notations to help you keep things straight.

Before we went electronic I simply created a spread sheet which indicated the frequency of visits, treatments, etc. I prefer the electronic method, it is safer and I always have it with me during a visit or at the office.

I grew up on a giant desk calendar and I still keep one in the office while we transition to EMR in case I fall off the face of the earth. I also keep a master binder with 60 day sheets that has all frequencies for SN and HHA, cath changes, labs and r/c's.

I don't know when I'll go all app but I can still visualize when things like labs are due after spending so many years with a updated desk calendar and a crazy photographic memory.

Google calendar has an option for businesses that is HIPAA compliant but it's not free. Our office is looking into it so we can actually put names on our calendars.
