Faxed me an order changing the insulin to carb ratio for one of my diabetic students. However, they failed to put a name, birthdate, or even MRN number on it. I asked them who it was for and if they could send me a copy with the patient identifiers on it. The person (nurse, I'm assuming) asked me if I had could call all the parents to see who had a recent ratio change and then call her back. Basically so she wouldn't have to look up these kids herself.

Working as a CM with the only employee owned Third Party Insurance group in the US I can let you know I get this each and every day from clinics, MD offices and hospitals. No name or I get 195 pages of 10 different people to everything in between. I just send it back citing HIPAA, ask for what ever I need-again. If it happens a lot with the same provider I call the office manager and speak to them and remind them about HIPAA.
