do you dilute meds before giving ivp through a running iv? usually not
how about an adapted iv? yes, more frequently, its nurse's discretion
can any nurse access a port at your facility? not without an order
do you use a preprinted order sheet for iv's--define type, no if you can use if for various things? flushes and volumes and frequency? we use computer charting and besides scheduled meds and prn meds we also have a mar for continuous meds, where we chart when new bags are hung. flushes for piv and central lines show up on the scheduled mar. this is more pertinent to central lines and picc's. if not how do you get an order--do you ask on admission or is it a protocol? would you be willing to share? if there is an order for a picc line or if an md inserts a cvc there is protocol for flushes, it is part of the order set for picc lines etc. what do you want, the order set for picc placement? it's computerized, but i may be able to get it.
who puts picc lines in at your institution? our clinical monitoring team or iv resource nurses who have been trained in picc insertion.
do you use prefilled flushes? yes!! the very best thing to have..saves tons of nursing time and is sterile...highly recommend!!

Hello
I am working on revising our current IV policies for PICC, central lines, and peripheral lines.
I have some questions that are not addressed in the standards I reviewed and wondered what you do at your facility.
Do you dilute meds before giving IVP through a running IV? how about an adapted IV?
Can any nurse access a port at your facility?
Do you use a preprinted order sheet for IV's--define type, if you can use if for various things? flushes and volumes and frequency? This is more pertinent to central lines and picc's. If not how do you get an order--do you ask on admission or is it a protocol? would you be willing to share?
Who puts PICC lines in at your institution?
Do you use prefilled flushes?
I think that is it for now--any input would be greatly appreciated.
