Is this a lowball offer?

I've already accepted another offer, but wanted to see what you guys thought about this offer. I'm a new grad still trying to navigate my way through the business side of things. I want to be more prepared next time around for contract negotiations. I did not accept this position but one office offered $50 per new patient and $25 per follow up. Its a pretty busy practice. The office manager predicted I would average a little over $500 per day and I could choose to work 4 or 5 days. Their benefit package was also lacking when compared to the other offers I got.

Does anyone get paid a flat fee per patient? Would this be a low ball offer if you are paid per patient?

Or office does a flat fee based on the patient visit if you choose to take that option. Though they tier it based on billing code. 213 to 215 are range from 25 to 30 PPT. New PT visits and annual physical are 32. We don't have to do this and to be fair as a new employee I would never work like that. But our longer term providers who want more control of their schedule like it. You don't have a working relationship with patients there and you will spend some time just getting your schedule up to par.

Where I work there are several opportunities for extra work. For all providers, we have a base salary, bonuses based on performance, hourly for call and working extra hours like at after hour or weekend clinics, several other options also.

And, as an incentive, for working especially busy areas, there was an option for getting paid by the pt for acute issues at $100/pt.
