Hi.I downloaded the visa screen handbook in their website, http://www.cgfns.org/files/pdf/apps/VS%20Handbook%20Web.pdf

Im having problem filling out my visa screen application form. i really have alot of questions!

1. In Preliminary information (1). about item letter D, i dont have any work experience yet. because i just graduated and passed NCLEX. what will i write here?

2. Other names (3). since i dont have any other name, should i leave them blank or just put N/A or none?

3. Occupational Visa Information (12). from h-1b, h-1c, 212®, TN(status), Permanent (green card). which one should i choose? i dont know any of them except for the permanent.

4. Educational Evaluation (14). Professional Education (b). i attended 3 schools. 2 of them are non-nursing schools. i didnt finished my course on the 2 non-nursing schools. i just transferred. tell me if this is the right thing to do - ill list all 3 schools in the form. then submit only the integrated TOR from my last school of which i graduated from.

5. English Language Proficiency (17). I havent taken up this exam yet. it says in the form that i can submit the vsc form prior to english exam. so i just have to leave them all blank? i wish there's a check box that says "havent took the exam yet" .

wow they accept fax?? im paying via credit card so if it will cost less to just fax my application to them it would be great!

hi! i have this question, i dont know if im gonna fill up the form "authoriztion to release information" since i dont have an agency and im processing my vsc on my own, do i have to fill this up?

im confused because someone told me that i should fill this form with the information about the state on which i got a license like for me its in new mexico, so i was told to fill this up with that because new mexico will be needing information about my application on visa screen. is this true?

i am also a foreign nurse who is duly licensed as an rn with the state of new mexico. now i am applying for the visacreen program of cgfns. according to their website a license validation from state of new mexico and and a transmittal of nclex passing test results is needed.

i was able to see in new mexico's website the license verification form for cgfns through this link [color=#003399]http://www.bon.state.nm.us/forms/verificationsfee.pdf. my question is that sending nmbon this form with proper payment of 30 dollars will cover both the license validation and the nclex passing test results? or do i need to send this nursys form via this link [color=#003399]https://www.nursys.com/includes/processing/verification.pdf to have my nclex passing test results separately transmitted to cgfns.

i am hoping that you can enlighten me on this one.

Hello,

I'm filling in visa screen application by myself too. As I see I need to send License Verification form to my BON.I passed Toefl for Florida BON. I've searche all FL BON site and couldn't find this form. Does anyone know where can I download it?

And another question, I found a hospital in NC, but haven't applied for endorsement yet. Do i need to write that I'll work in another state in VSC application form?

As I understand I need to send to Florida Bon a page from visa screen handbook named Request for Validation of License. But there's no information about payment. How can I pay for this form to my Bon?

Did FL BON actually gave you a license number as they also have a US SSN req't. I would contact FL BON thru e-mail or phone to be sure but I reckon it would be the same fee they usually ask for if doing a regular license verification to another State BON.
