Hello,

Can you guys share your IHCPs?  I’ve been trying to develop some for my students and I’m a bit stuck.  
My families aren’t great with providing information so it’s hard to get in depth assessments.  Just trying to see what other nurses are doing to develop plans with limited info?  Specifically looking for asthma, allergies, seizures, and migraines.

Hi I think you're referring to what we call IHP's (Individual Health Plan)? We only write them for students with allergies and Epi-pens in the clinic, seizures with diastate in the clinic,  diabetics students that we provide care for, and serious cardiac issues as well as case by case bases. We only keep them to this amount because the inhalers alone would take us all year to write and IHP on. We let the teachers know about medical students in a notification box that pops up when you research a student (if they have medical problems). Hope this helps and wishing you a good year!

We use our Dr's orders as the IHP.
Asthma Action plan = care plan
Food allergy action plan = care plan plan

Seizure plan = care plan

The only ones that I write up are the T1D kids and if they have some other unique/unusual situation.

Okay that’s what I was doing in the past.  I used what the doctor gave us but I just was curious to see what other nurses were doing.  Thank you!

What EHR do you use? Because Skyward has some template IHPs that you can fill in if you do not get a good set from the doctor.

Also NASN has "Individualized Healthcare Plans for the School Nurse." My version is old so it's all print BUT I am sure they have an electronic version now.

Also remember: the simpler, the better; and if it's written we have to do it.
