I spoke with the owner. I explained that the schedule changing everyday after I got to the office, having to re-sync, then get supplies and teaching materials was taking 1 to 2 hours before getting out the door then seeing different patients everyday, especially doing DC's was ridiculous. It's difficult on pt's you don't know and time consuming. Everyday a different town, having to print mapquests for every patient. gett
She said you can't make money that way. (um, yeah).
She said you're supposed to be in one area and see the same patients each week. You have your own patients and you case manage your own patients. See 5-6 patients a day and knowing them you can knock them out in about 30 minutes each and they're all in one area. If you're done by 1:00 the rest of the day is yours. She said "you know I was looking at a chart and noticed that 6 different nurses had seen that patient and I thought that was really odd, and so wrong, I asked the DON about it but never got a straight answer". The DON couldn't give her any reasons for the nurse with all the OT either, hum...

DutchgirlRN, I really hope things work out for you.
I had read alot of your post before the changes took place at your office, and they inspired me to try HH before giving up on nursing completely. Now I have a HH job I LOVE.
Wishing the same for you!

I'm glad you had a chat with the owner.
Now the only thing I can think of about why that nurse took so long on visits hence the OT is maybe she has ADD or OCD? Something like that? Which would also make sense for your DON to make that statement about "if you knew the reasons behind it you'd have a different opinion" Either that or she's sick with something.
BUT, how is she doing timewise now with visits? Because if she's ok with time now, then she's is FOS.
But anyway, I hope something good comes of your talk, I really do. Keep us updated.

I'm very glad you had the opportunity to speak privately with the owner. It sounds as though she has been misled by your manager. Good for you for setting the record straight. And I, too, hope this turns out well for you.
Take care.

Dutchgirl,
I am glad, too, that you got to meet with the Agency's owner. I am not surprised at her reaction, for I have seen my share of Agency DON's and Managers lying about their operation of a branch office in order to paint a rosy picture for them. Years ago I went to work at one HHA as a QI nurse and immediately found massive documentation and billing issues which were being covered up by the DON as well as the QI supervisor who were close friends. When I told the Owner what was going on, she HIT the ceiling. The following day, she brought her son and daughter (who were co-owners) with her to that Branch Office and CLEANED HOUSE. Within 15 minutes both the DON and her buddy were gone, and the office was immediately reorganized. I think the moral of this situation (as well as yours), is that the Owners of these Agencies need to make it their business to know what is going on in the day-to-day operation of their branches...to ensure that problems like the one yours is having doesn't happen to begin with.

Now being per visit she has miraculously sped up and now instead of trying to hog all the admissions she is trying to hog the regular visits. Pretty telling, eh? If I were the boss, her a** would be past history. :angryfire
Maybe the owner will intervene and make it happen but I don't think she will at this particular point.
Oh well......Life Goes On......

Amen, CseMgr1! Evidently the owner was serious once she knew the truth, and thanked those of us who were brave enough to stand up and tell her.
She also CLEANED HOUSE. The DON and her buddy the care manager got fired.

Well it's about time!!! That is what should have happened a long time ago instead of all this other nonsense. Maybe you could have kept your job.
Catti, that is sooo true!! Notice how I'm banging my head on the wall due to all the frustration? It could have all been avoided. So true!! Thursday the DON fired me. I said "OK". She called HR and told them. A few minutes later I got a call on my cell from the owner telling me I was not fired. The owner also fired the nurse with the excessive OT.
This is just a shame. Like you, I used to love Home Health Nursing: the autonomy, bonding with patients and their families and earning the trust of the medical community who at one time didn't want us touching their patients. I could only stand back and watch, as the agencies I worked for began to disintergrate due to not only the mismanagment which you have described in your posts, but also because of politics and greed. I became so disillusioned with the industry that I finally quit last year...and for good.
