TOEFL is changing like IELTS SOON. I think the best test for students is IELTS.
I want to study in America which one I should take course.
Who can you tell me to answer this question?

TOEFL is changing September 2005. Preparation for either exam is essentially the same, only difference is that ETS was the American exam, and the IELTS series was origianlly designed for Australia and the U.K.
Personally, I think that the IELTS exam is a better exam from a hiring standpoint as it gives a truer study of how well someone will do with English.
You can actually pass the TOEFL exam and not be able to speak a word of English...............that is why TSE is so important, as well as the speaking band of the IELTS.
My suggestion: Worry more about your conversational skills and the rest will follow. If you just try to prepare for an exam, that is not the way to learn English. The vocabulary used in the exams is not what is used on a routine day for anyone. Get your speaking and listening skills up to those of a native speaker, then and only then, prepare for the exams. You will do much better that way..............

I am writing on behalf of my fiance, who is an RN from Turkey, she is currently working in the Bay Area, California as a home care nurse.

I dont think there are courses for IELTS but TOEFL. I took a TOEFL course and took the exam. You should take computer based TOEFL and get at least 540. Then you need to take an exam for speaking as well. But if you take IELTS, you dont need to take an exam for speaking. Thats what I know.

Yes, you do take an exam for speaking with IELTS. It is just included with the exam. You must take the academic branch with the speaking section.
Sorry, but with either English testing, there will be a speaking part.
There is no time limit as to how long you have. You need to have English completed for your Visa Screen Certificate.
There is an excellent program that is available on-line for preparing for the IELTS exam. It can be found at http://www.scottsenglish.com. The program is based out of Australia, where IELTS reigns and they have an excellent program. Yuo can pay by the month or for a three month program, and access it as much as you want. I have been quite impressed with their program.
Computer based passing score is 207, pencil and paper is 540. They are two different exams.
