Pt had surgery, so she has a large chest incision, but it doing very very well. shes an older patient (over 80). She is already ambulating on her own, and doing great. I can't really think of where to start, because shes going home. I know shes at risk for falls of course, but I cant really think of something good to start out with. The only thing I really thought might be good is risk for infection since she has a large chest wound as well as a large incision on her lower leg. Any help??

I would definitely consider risk for infection. Is her mobility limited d/t the leg incision? Risk for DVT, perhaps. How is her nutrition?

Indeed, risk for impaired mobility and nutrition imbalances. Also, consider knowledge deficits, pain, impaired skin integrity, risk for impaired ventilation, etc.
