i need help.. I'm doing a ncp right now and my problem is about the related factor for altered tissue perfusion .. our patient was experiencing difficulty of breathing .. and I'm thinking that i can relate the altered tissue perfusion on difficulty of breathing.. is that right? can i relate DOB to altered tissue perfusion? this is for our case study i need answer thanks. and also can u give me the scientific explanation of altered tissue perfusion related to difficulty of breathing.. thanks

oh, i need altered tissue perfusion, because that problem was the 1 being told to us to do... it is possible to relate DOB to that?

ghuk- you may want to look over your case study again. Was the patient's hemoglobin low, by any chance? In other words, look at why the patient is short of air, and you may find your answer there.

What you are attempting to say is that the patient's DOB is the cause of the Altered Tissue Perfusion and that is not logical. Altered Tissue Perfusion means that the circulation to the tissues is not getting enough oxygen or nourishment. The older definition of the diagnosis, Ineffective Tissue Perfusion, was: decrease in oxygen resulting in the failure to nourish the tissues at the capillary level (page 228, NANDA-I Nursing Diagnoses: Definitions & Classification 2007-2008). Blood brings not only oxygen but also nutrients (nourishment) to the tissues. So, using DOB as a cause of Altered Tissue Perfusion fails. DOB can, however, be a symptom of Altered Tissue Perfusion of the lung tissue, especially when there is a pulmonary embolism or pulmonary edema present.

The definitions relating to tissue perfusion have been recently changed by NANDA and the label has been made more specific: Ineffective Peripheral Tissue Perfusion and its definition is now decrease in blood circulation to the periphery that may compromise health. It makes no sense that DOB can cause any kind of physical interruption to the blood circulating to the tissues. In addition, Ineffective Peripheral Tissue, cardiopulmonary (the old diagnosis)is no longer a recommended diagnosis to be used when the patient has a pulmonary embolism or pulmonary edema. One of the respiratory, cardiac or activity diagnoses should be used. To diagnose them, you need to know what the patient's underlying pathophysiology of their medical disease process is and their symptoms. Since you have not given that, I cannot help you much more.

I must point out to you that diagnosing is based upon the both the symptoms that the patient is exhibiting (DOB is a symptom) and the pathophysiology of their medical condition (you did not give any of this information). You need to go back and look more deeply at your assessment information before attempting to diagnose.
