How to request for fingerprint card to IDFPR?

I am staying in florida. I want to request for fingerprint card (for nclex rn illinois bon) to the IDFPR. How can i send an email request on the profession page of the department website at http://www.idfpr.com?. 0R is it that i have to take my personal email and compose an email request for fingerprint card and send it to http://www.idfpr.com?

How much is the fee as money order to be sent to the cts along with the 4 page application? Is it $91 only OR $91 plus $200 (NCSBN examination)?

Finally, the 4 page application form, CT-NUR,FP-NUR, fees, Are these the forms I have to send to the CTS, or anything else more?
