Help With Nursing Care Plans

Hello all I am in need of help with a nursing care plan and I have no idea where to start. This is my first care plan and I need assistance because it was not explained good.

My patient:

  • 78 year old female
  • Admitting dx: cp and copd

Complaint: "Chest pain when I first came in", shortness of breath

History:

  • Smoker
  • Stents
  • CVA
  • COPD
  • Dizziness
  • Noncompliance

Additional info: green dot precautions(green socks)

My problem is I have to have three nursing dx and I was only with this patient for one day and she was discharged.

Please help I need dx, patient goal, interventions, rationale
