help for these questions

i have two questions from nsna's review cd and the answer is different as kaplan. please help me to understanding more.

a nurse coming off a night shift rotation on a psychiatric unit. most of the clients have slept through the night so you have little visual recognition of your clients. in order to properly identify your clients prior to administering medication on the day shift. it would be important for you to:

a) call out their full name and ask them to verity it

b) only give medication to clients wearing armbands

c) ask the client to state their full name

d) verity their full name with another nurse

the answer is c.

what i thought the answer is b. because this is psychiatric unit, client may not kwon their name.

a ten-year-old boy enters the office if the school nurse and says he just had glass fly into his eye from a broken bottle. the best initial action of the nurse to take is;

a) call the boy's parents to take him to an ophthalmologist

b) position the boy in a semi-fowler's position and cover both eyes with eye shields

c) evert the upper lid with a sterile swab and look for any penetrating wounds or loose pierces of glass

d) irrigate the eye gently with saline solution

the answer is c.

what i thought the answer is b. because the nurse should not do any thing to make more hurt to the boy's eye.

please please help me to explain these answers. why?

Well in the second questions, I would think the answer is correct because the nurse has to assess before she does anything else, and if she was to just cover both eyes then she is taking action before assuring that there is a problem.

In the first question, I dont get that answer either, why would you as a psyche patient to give you there name before passing them medication, they can give somebody else's name. I need clarifying on that too.

Its correct, why? the nurse in this case have to avoid complications from a big piece of glass visible that can cause serious injures in the inner structures, just the first step, before clothes both eyes as you said, that is toattly correct but not a prioprity in this case.

Very important be focus in the situation that you are facing, what is more safe for the patient all the time.

the first one is c because what i learn't is that you answer based on what you know. you need to realise that the psy. patient is in a unit with other patients so he may very well be mentally stable enough to state his full name. there is no information to state he was isolated or out of control and he may have been on an acute ward who knows.Also do not explore the possibilities in questions like this because you end up adding what is not necessary. i want you to read it again, this time do not explore. work with what you know. did that help? i used to do this and it was what had me confused.

right because a pyc pt mybe just depressed or have a borderline personality it does not sayanything about the pt. being psycotic. dont read into it you cant think what if??? a pt. is a pt. a pyc pt is the same as a heart pt when it comes to physical things. you should always ask the pt. even a confussed pt usually can remember there name. loc's might know there name but just doesnt know where they r or how they got there this usually the case. just dont dont read into they question.
