Help prioritizing nursing diagnoses

Here are a list of nursing diagnoses i came up with for my patient, but i am having trouble prioritizing them. Can any help me organize them from the most important to the least important. THANKS!!!

Acute pain r/t effects of chemotherapy and cancer s/t to burns, mucositis, and breast cancer.

Impaired skin integrity r/t effects of chemotherapy s/t burns, mucositis, and breast cancer.

Impaired physical mobility r/t weakness and muscle loss

Constipation r/t effects of narcortic

Risk for disturbed body image r/t changes in appearance s/t mastectomy and burns

Specializes in med/surg, telemetry, IV therapy, mgmt.

    • the burns, mucositis, and breast cancer are the cause of the pain because they result in injury to the tissues producing the pain. the chemotherapy has nothing to do with producing the pain and you need to remove it as an etiology. your instructors may not accept "cancer" since it may be viewed as a medical diagnosis. you may need to re-word it.

  • what caused this patient's open skin areas? a burn as in they were exposed to a flame or a caustic chemical? mucositis is not the cause of open skin areas, but the result. breast cancer does not cause open skin areas. open skin areas are caused by things that break down the skin barrier in some way.

Always think Maslow

  • "What caused this patient's open skin areas? A burn as in they were exposed to a flame or a caustic chemical? Mucositis is not the cause of open skin areas, but the result. Breast cancer does not cause open skin areas. Open skin areas are caused by things that break down the skin barrier in some way. "

I have seen a number of pts w/ advanced cancers (especially breast) which have broken through the skin causing painful open areas.

to the op, the burns and mucosistis were most likey caused by radiation tx, not chemo.
