I had a pediatric patient yesterday who was 10. He has down's syndrome and was hospitalized with gastritis but they did a CT scan and found out he had ulcers in his esophagus and stomach. He was transferred to a larger facility. I have to do a care plan on this patient..
The diagnosis I was thinking of was either, Risk for Dehydration, Risk for Pain, or something related to the down's syndrome like risk of developmental delay.. I was a little confused about what I should list for the AEB..
What diagnosis would be better?

Your priority diagnoses have to relate to his medical diagnosis. He has a lot going on related to his gastric and esophageal ulcers that has nothing to do with his developmental status. (And for him, there's no RISK of developmental delay, he HAS developmental delay, however mild.) While he's at risk for dehydration IF oral fluids are the only source, but I wouldn't put that at the top of the list. Think about what you know about gastric ulcers. What other problems can be associated with them? What nursing diagnoses fit?

You have to also consider his mental status and how much he understands . some tests caan be very frightening for any child . What is different about this little guy?
