-- A 10 year old Italian male was referred to having problems with academic performance in school. For more than a year, his grades have dropped from A's to B's to D's. The patient and his mother were at loss as to why he begun to do poorly in school. He reported he had not been able to concentrate on his homework due to frequently feeling sick to this stomach when studying. When asked what helped him keep an A average, he states his father used to help him with his homework and now his father is dead.
what would be a priority nursing diagnosis?
i came up with ineffective resilience, but i cannot seem to find the right choice for related to ......, as for manifested by dropping of grades, lack of concentration

how about demographics that increase chance of maladjustment...his demographics are he went from a two parent household to a one parent d/t the death of his dad.

Do you use mind maps - they're a pain but they do force you to "see your patient." If not, try brainstorming by listing all of the subjective and objective data first (your assessment). Then take this evidence and connect it to a nursing dx. Never work backwards. I find that by doing this I usually come up with all the r/t and aeb then find a nursing dx that fits those pieces of data. Good luck.

Think about how you would feel if your parent/significant other died. The sadness. Are his coping mechanisms ineffective? Is he suffering from an interrupted family process? Is the boy grieving? Is his grieving complicated? Is the little guy just plain depressed. Does he feel a sense of hopelessness with his father not there? I his low self esteem situational? All of this in response to the loss of his father?
