First of you come in to your patient room and assess them;are the fully awake,alert,talk to them asked them do they know their name,do they know where they are,do they know what date is it?? Observe them;are their eyes fully open,are they lethargic? Do they seem confused..Next you ask them to grasp your hand (bilaterally) Is their grasp strong or week? Does it very or is it the same on both side of the body,next asked them to push each leg against your hand,are they able to do that...next ask them do they feel any weakness in some part of the body,any abnormal sensations,tingling,numbness?? Touch to asses the temperature of the skin,look to assess color.
