HELP NEEDED ASAP, with a bath program that work and can be tracked.

I am a treatment nurse in a LTC facility. They recently disbanded our bath team due to low census. We have 8 CNA's on first shift, 8 on second and 6 on third. That is if no one calls out (that's a problem all on it's own). I am noticing baths are not getting done, no nail care or mouth care. We currently use bath sheets and the floor nurses are to sign them before they go in the binder. They are getting signed but no one is checking if they are truly getting done. Problem also with these sheets are, we have 92 residents that should be bathed three times a week. So that's 276 sheets of papers every week. What am I supposed to do with all of this to track or check if a resident received a bath requires looking thru all those bath sheets. Any suggestions of a web site, any forms or a guide, or just some ideas of what works other places. Thanks in advance. ...,

Ok, I'll give it a go. You could maybe create a monthly paper for each resident and put them alphabetically in a binder (with those handy alphabet tabs) and have staff sign off each bath day. That would save paper, and create easier access to see when someone last had a bath. At a glance you could easily see that Mrs. Smith didn't miss a shower all month.

We have eMar, so CNA's sign off there that showers were completed or not. We do "shower sheets" on Monday and Tuesday with complete skin checks. The CNA's give those to the nurses and it's the nurse's responsibility to follow up on any skin issues found. Those are then signed off by the nurse and turned in to the unit supervisor. I have no idea what happens to them from there. I have a feeling they go into the circular file.

Since there are no showers on Sundays, we do nail care then. There are nail care sheets listing all of the day shift showers for days to do, and one for evenings. The CNA's are responsible for completing the nail care and signing off. The nurse's then sign off that the CNA's did what they were supposed to do. Those completed sheets then go to the supervisor. Again, circular filed I'm sure. I don't know. Maybe she has a binder for them...

Anyway, oral care should be completed no matter what, and while the CNA's chart in eMAR that it was done, it's really up to the nurses to check and make sure. We have no paper tracking for that.

How many patients does each CNA have on a given shift?

Really I think no one is checking and they are not being held accountable. No it not all residents or all CNAs. But I do find it is the more difficult or bed bound residents that are more likely to go without a decent and regular bath.

We do ONE bath per week(unless the resident requests otherwise) Between beauty shop appointment and am and pm care this is adequate......We do our weekly skin checks with the bath,nail care is part of the bath....It all gets done
