Call the NJ BON. They will be able to give you the instructions you need. Their number is on the BON website.

Here is the link to the application. Good luck, it appears to be quite the PITA, but I wouldn't expect anything less from NJ.

I made the big mistake of not renewing my NJ license when I worked in Pa. I was going through a divorce, I was working for an agency, all my assignments were in Pa and could not afford both licenses. This was back in 2007-2008. If I remember correctly- I had to write a letter explaining what I had been doing, send documentation of such, send a copy of my Pa license and why I did not renew and pay for every year( the back years- which I think was 3 years) for when the license was inactive. I think it cost me approx $495.00 total, to renew my NJ RN license that year- the BON has a chart for the fees and NJ takes a long time. I think NJ is the longest or one of the longest states for license endorsement.
NJ is a buggar- it's just like traveling the bridges from Jersey to Pa or Delaware- it's free to get in Jersey but it will cost you to leave, you can sell and buy another house in Jersey- costs little to nothing, but sell your Jersey home and move to another state- it costs you big time money!!!
