Hello - I am a nurse manager of 14 years and a nurse of 23 years. I am currently in the part of my MSN/MBA/HCM course. Thought I would check in to the allnurses.com.
Any of you having any success with med reconcilliation?

I'm sure if you post your question in the General Nursing section of this website, you'll get some answers. This board here is just for introductions.
I'm having a moderate amount of success with it on discharge. The key for me it to catch the docs while they are on the unit discharging the patient and ask for it. I usually in the morning print out med-recs on people I hear are going home.
Other nurses aren't as diligent and are ignoring them at discharge and doing things the old way. There haven't been any repurcutions of this yet, but I'm sure when JACHO comes next it's going to suddenly be a big deal.
On admission we print them out for their home meds and they are ignored maybe 75% of the time, so it's a failure here.
