Health Office Passes

I've been brainstorming some ideas to change our nurse passes for the next school year. Currently ours has students name, todays date, arrival and leave times, teachers name and a place for me to sign.  

I am thinking about adding in a section that has the teacher checkmark some strategies they have tried already in class kind of a reminder to teacher and student first too of what they need to try before just sending them into my office.

Im trying to prevent the " I fell and hurt my leg back in 2012" or "my head hurts" when they havent eaten, drank any water before coming into the health office, hopefully this makes sense what im trying to verbalize.

any tips on what you have on your passes?

I believe I got mine from Teacher's Pay Teacher's. It is simple but works well in my school.  It has name, date time then check boxes for the teachers to check off what the student is complaining about, and a spot for the teacher to sign.
