Health History Forms -- New School Nurse

Hello!

I am a new school nurse at a small parochial PK-8 school. I am new to the world of school nursing, after having 2.5 years of adult ICU experience. Needless to say, it's a big change, but this is an area I am extremely passionate about and eager to take on all the new-ness and challenges it may bring. That being said, this school has also never had a school nurse before, so I've had a lot of organizational work set out before me.

Currently, they don't have an organized way of obtaining health information from students. The only forms I currently have on file are Prescriber/Parent Authorization Forms for students with prescribed meds needing to be administered at school (mostly asthmatics with inhalers). I have received a few notices of allergies from parents (informally, or on the previously mentioned form for those with prescribed meds), but otherwise, I feel there is a wealth of information lacking. I am working on compiling some forms for ALL students to complete that will include all the information I desire--past medical history, allergies, ALL meds a child is taking (not simply those that I will be responsible for administering at school), emergency contact, etc. I am also very much desiring a similar form for all staff/teachers to complete, as I am just as much responsible for their health needs as the students.

I've been scouring the internet for such forms for ideas (or to find the ideal form to simply copy!), and I thought it wise to check in with those of you who are experienced in this field who may have access to some resources that could benefit me. How do you handle this at your school?

Also, any other advice you would like to willingly toss my way would be greatly appreciated!
