Health Care Proxy

Hi Guys!

Well for one of my homework assignements I have to make a healthcare proxy for myself............to be honest I have thought about it but to actually put it in writing it is completely different. There are so many different options and choices and my original thought of "just pull the plug" has now got me thinking deeper about it.

Anyways....... the wording has gotten me completely confused.

Basically if I am at this age 28 then I want as much medical intervention as needed to get me well if I am in an accident etc

But at 88 I dont want to be touched if I am terminal.

So anyone know how to help me word this. I do not want to make any errors and I have googled and researched and I think I am more confused than when I started.
