Tracking Bird Flu Cases

Bird flu deserves its own thread for tracking suspected and confirmed cases. It's not the pandemic virus, but it is still an ongoing and significant threat because of its virulence. As Margaret Chan, the Director General of the WHO says, we do not know how H5N1 will react under pressure from the pandemic virus, H1N1. All of the countries with endemic H5N1 are also reporting cases of swine flu. As everyone should know by now, the new concern revolves around what will happen if the two viruses co-infect the same host. Flu viruses exchange genetic material with each other all of the time. Right now we have a pandemic virus that is very transmissible, but does not kill the majority of people infected. Bird flu, on the other hand has a case fatality ratio (CFR) of around 60%.

In the past year, something different began to happen in Egypt. We started seeing many cases of young toddlers infected with mild cases of bird flu there. But, we also saw some older children and adults that had fatal cases. No one has yet explained why the disease began to change in Egypt. Just prior to the beginning of the swine flu cases appearing in the US and Mexico, the WHO was going to send in a team to Egypt to investigate why this was so. And, then the swine flu pandemic began in earnest, leaving this question unanswered. Why this situation has changed in Egypt but not in Indonesia, is most likely very important information.

Egypt continues to try to isolate swine flu cases. At some point, they won't be able to contain that virus, and will have to bow to the inevitable. Nobody knows what will happen then, but the Egyptians are clearly concerned. Remember, this is the country that slaughtered its entire pig population a few months ago to international dismay. Maybe they were right to do so, as we are now finding swine flu in pigs in Canada, Argentina and Australia. Pigs, the perfect mixing vessels for influenzas, may have been harboring the current swine flu for years undetected because they are almost never tested in most countries unless they are sick. This lack of disease surveillance is now being compensated for by a heightened awareness and increased testing of pig herds in many countries.

With all of that being said, here is a translation regarding a new case of bird flu, H5N1 in another child in Egypt. They do not state the child's age in this article, and some translations are saying that this is a male child. They would have isolated her anyway with bird flu, but with swine flu also in Egypt, they are likely to be very worried. Descriptions of these cases almost always mention dead birds being found in the same location. That of course, is going to be helpful in determing which flu they are dealing with at least for now, but maybe not later...

The Ministry of Health on Sunday, a new human case of bird flu, a girl from a province of Kafr el-Sheikh for a total of 82 case of injuries so far.

A statement by the Ministry of Health said on Sunday that the new situation of the girl child is Muhammad Mustafa Ahmed Rania from the village of "Sidi Ghazi," the Department of Kafr El-Sheikh governorate of Kafr el-Sheikh.

I continue to be fascinated by this disease, and I sincerely hope that it never learns to become more transmissible to humans.

In contrast to the swine flu, which killed only 1 out of every 2,000 people who got it, the avian flu kills about 3 out of 5.

Since 2003, there have been 508 cases around the world, with 302 deaths. Only three countries-Indonesia, Egypt and Vietnam-have reported more than one or two a year recently. It is more often found in poultry flocks.

The government in Indonesia recently released details of two deaths. One was a 35-year-old Jakarta man who died after 11 days despite prompt hospitalization. The second was a 40-year-old woman who died a week after falling ill. For unknown reasons, a greater percentage die of the flu in Indonesia than elsewhere.

Japan will cull about 20,000 chickens at a farm in the western prefecture of Shimane after the country's first case of avian influenza since 2009 was discovered by the government today.

The government has asked the farm operator, which also raises about 3,300 chicks, not to move the birds out of the area to contain the disease, Yuji Kawakami at Shimane Prefecture's livestock office said today by phone. The owner of the egg- laying chicken farm in Yasugi city found five dead birds yesterday, he said.

The government has asked farms within a radius of 10 kilometers from the Yasugi farm not to move birds, according to the Ministry of Agriculture, Forestry and Fisheries.

Japan: Gearing Up To Tackle Bird Flu

For the first time in more than a year, Japan yesterday announced the detection of what they believe to be the highly pathogenic H5N1 virus on their soil.

While early PCR testing showed H5N1, final laboratory confirmation is still awaited.

Today, the Mainichi Daily News is reporting that in the face of this latest outbreak, Prime Minister Naoto Kan has ordered officials to do everything in their power to prevent the further spread of the virus.

Indonesia

Indonesia: The Long Wait For Test Results

Not always easy to find the H5N1 virus even though it may be there. There is quite a history of this little problem occurring when docs were pretty certain that bird flu had killed the patient, and this was in countries other than Indonesia. Sometimes it took an autopsy to establish cause, and find the virus. But, they don't do autopsies in Indonesia so if this woman dies, they may never know what killed her. And, then there are the other issues which this link addresses well.

On November 22nd, a 21-year-old woman from West Java, Indonesia was admitted to Hasan Sadikin (RSHS) hospital with serious respiratory symptoms, and was almost immediately placed on a ventilator.

The tentative diagnosis was possible H5N1, or bird flu, infection. Laboratory tests were ordered to either confirm, or rule out, the virus (see Bandung, Indonesia: Bird Flu Suspect).

Twelve days later, and after multiple laboratory tests (results undisclosed), the young woman remains on a ventilator (although somewhat improved) and doctors are still unable to say whether she has the H5N1 virus.

Indonesia: Bandung Patient H5N1 Positive

...received an email from a Ministry of Health official in Indonesia saying that the 21-year-old woman from West Java, Indonesia admitted to Hasan Sadikin (RSHS) back on November 22nd had tested positive for the H5N1 virus.

Testing for the virus can be difficult. The good news is that Depkes - the Ministry of Health - publicly acknowledged the positive result once they had it.

As the northern hemisphere moves into winter we are already seeing an uptick in the number of H5N1 reports in humans, wild birds, and poultry. This is to be expected, and has been the pattern since the virus re-emerged in 2003.

I did not know that the South Koreans were monitoring so vigorously. They have been very agressive in wiping out all birds and animals in infected zones even if they were pets as we have seen in the past. They seem determined not to allow this virus to become entrenched in the environment.

...the discovery of two dead owls leads one to suspect there are more infected birds and/or animals in the region.

While the detection of a few wild birds with H5N1 in one week in Korea is hardly alarming, it is a reminder that the virus continues to exist, and spread, in the wild.

Interesting. I lived in Dongducheon in 1976 just outside of Camp Casey years ago when my ex was stationed there on an unaccompanied tour. It would be the wet market there that would concern me. That would be the most likely place for any exposure other than the farms where the chickens are raised.

What are the precautions saying to do or not do? The Koreans are very agressive when it comes to trying to wipe out this virus. They can't keep it from reappearing but they will do whatever it takes and more than that to control it.

Two days after the chickens at Nawaganga Poultry Farm in Mangalpur-3 showed symptoms of the disease, rapid test had revealed H5N1 avian influenza virus on October 27. The symptoms were confirmed by real time Polymerase Chain Reaction in a central veterinary laboratory on October 28. The sample was tested positive in Veterinary Laboratories Agency (VLA), in the United Kingdom on November 19.

Sources claimed that the Cabinet had refused to declare the outbreak even after it was informed about it in October-end.

Dr Ram Krishna Khatiwada, programme director, Directorate of Animal Health, in his capacity as the communication officer from Nepal to World Organisation for Animal Health (OIE), reported the case to the OIE only on December 12. Around 11,500 poultry were destroyed by the government agency at the poultry farm during this recent outbreak.

Last year also, the government had refused to disclose information about bird flu outbreak in different parts of the country. The government was forced to disclose the outbreak a month later after THT informed the public about the possible danger.

a case here, a case there, the virus goes on infecting humans/ poultry or other birds/other mammals in the different countries that have the bad luck to have viral exposure. whether the virus is smuggled in the body of a duck or chicken, or flew in via the body of wild bird, once it is in the environment, you have to agressively go after it like the japanese and the south koreans do. who knows what china is doing though, and they are most likely the ultimate exporter of this virus.

how did the virus get to egypt? impossible to eradicate it now...

most cases in humans are probably not severe in egypt, (just a guess that most are not even tested but kids are) since the toddlers infected are not dying, one would think that the adults must be infected also. this gives the virus lots of opportunity to adapt humans and evolve.

shaheen went noted that the women entered a local hospital with a high fever and flu symptoms after being exposed to household birds suspected of carrying the disease.

Japan

H5N1: Japan: H5N1 detected in swan

Tundra swans most likely have migrated from futher north of Japan. This would be an example of the virus having migrated into the area in the body of a species of swan. The mute swans most likely have not migrated but were infected locally possibly by swimming in waters infected by birds migrating into the area if what I know about swans is correct.

How the poultry got infected is anyone's guess, but there is probably a logical answer.

The Tottori prefectural government unveiled the result of a test on the wild, young tundra swan, which was found reeling on the balcony of a residential house in Yonago on Dec 4 and died the following day.

The location is within 10 kilometers of a poultry farm in neighboring Yasugi, where over 20,000 chickens were culled by early this month after city officials confirmed an outbreak of highly pathogenic avian flu.

Meanwhile, officials of Toyama Prefecture are conducting a further investigation into suspected cases of bird flu infection after a swan was spotted swimming feebly Saturday in a park where two swans were found dead earlier in the week.

It was the second time a weakened mute swan was discovered in as many days in a moat inside the premises of the park in Takaoka city after the dead swans were found there on Thursday, one of which subsequently tested positive for bird flu in a preliminary check.
