The only form that was attached was the medical clearance form, as far as I know, documents needed to be submitted through the student portal.

Can you clarify what "both" were to be filled out? That would be the medical clearance form and...?

The HIPAA Form and the Medical release were attached to the e-mail. You are to schedule an appointment to receive a physical and give the physician the medical release form that is to be filled out. The HIPAA form is to be signed. Both are to be uploaded by December 1st

Thanks for the information! I have not received the HIPPA form through email or through e-doc (from what it said on the email), I'll contact my counselor right away.

Tuesdays: Nothing

Wednesdays: 8-12 (Foundations of Nursing)

Thursdays: 8-11 (Health Assessment)

Fridays: 8-11 (Pharmacology)

2-5 (Intro to Professional Nursing)

I just looked and I saw that I have mine! It looks like:

Monday: Intro to Professional Nursing 8-11

Foundation of Nursing Clinical

Tuesday: Foundation of Nursing 8-12

Wednesday: No Classes

Thursday: Health Assessment 8-11

Friday: Pharmacology 8-11

I'm so happy with this schedule! I got placed in Banner Boswell, where did you get placed?
