I'm very confused by your question. Is this a homework assignment? Are you in nursing school? What year are you in? What exactly is your assignment? The steps that you have listed describe the nursing process, the steps by which we, as nurses, assess out patient, decide what they need, come up with a plan for their care, put it into practice, and then evaluate whether that plan was effective or not.
Please provide more information about your assignment, what you have done so far, and what, specifically, you need help with so that we can better answer your questions and guide you in the right direction.

i`m working in SCBU (special care baby unite ) for 8 years and they choose me to be preceptors
and they give me assignment for what i will do for the new preceptee and they need my goal and how i will assess , my objective ,implementation and evaluation for each goal

Here's where I would start: Make a list of the most important information/skills that a nurse needs to know to work in your unit. Some things I might include are - how to do an admission, how to do a discharge, most common medication, most common procedures, taking accurate vital signs, and symptoms that could indicate a problem (such as retractions when breathing for a baby).
Then your goals would be for your preceptee to be competent in those areas by the end of the rotation. You could evaluate with a written quiz that you make up or by observing the student perform the skill.

I'm having a hard time understanding what you need. Are you the student, or are you the nurse who will be the preceptor?
If you are a student, you should be learning this in school. If you are a nurse and have been asked to be a preceptor, you'd better know this already! So tell me what I'm missing.
I don't work in a SCBU, so I don't know what types of procedures and skills you perform on your unit. Nor do I know what expectations your managers have for this kind of assignment.
This is sounding more like a homework assignment than something that a preceptor gets asked to do. Why don't you ask your nurse manager (or whoever chose you to precept) for examples of how you should format this project. I'd also ask them why it's your responsibility to come up with a "care plan" for precepting and not the job of the instructors of the nurse educators.
