Getting a Job at Hospital Where You Previously Turned Down an Offer

So, the title pretty much says it all.  If you turn down a job offer, will that hospital likely not extend any more job offers to you in the future? 

The type of unit I want to get into is typically competitive so there really isn't any chance of being offered a job because they are desperate.

It probably varies by geographical region and hospital, but what I've seen is the more important aspect is how qualified you are for the position versus other applicants.
