I would like to know what other facilities are doing re: monitoring the use of psychoactive medication in the elderly.

Ex: forms, what is recorded, frequency of monitoring, audits, etc. thanks, judy

We monitor our psychoactives on the MAR q shift, any behaviors are notated with a "y" and addressed in the NN. Side effects are also monitored. If it is a continous problem, then it is documented as such so that the nurses don't have to rechart it qshift. We've also used seperate forms, but this is the easiest way for us.

In response to the question of monitoring psychoactive medications, our facility received a deficiency two years ago (NY) because of an error in documentation in using a psychotropic drug. Since then we have used a form that is right in front of the nurses notes that provides a place for each shift to document behaviors. It also has a section for information on the drug, expected benefits and side effects to monitor for. It has worked well, the docs like it and the state has had no more complaints. If you would like to contact me, I would be willing to mail a copy of what we developed. This does not preclude charting, but makes it easier to see long-term effects and ensures monitoring.
