Not sure what kind of help you need but this is basically what I do and maybe others can expand on this:

Medical Diagnosis in the middle of the page

Underneath this list your primary assessments for this patient

Surround the medical diagnosis with nursing diagnoses that you believe are key for this patient and number them according to level of import

Underneath each nursing diagnosis list patient subjective and objective information that supports your diagnosis

There's a really good program that I used for micro called Inspiration. This one's not free though.
