Can you inject IM into vastus lateralis in an adult? Why does everywhere I look say to inject into the upper outer buttock quadrant? I hate to use this site because of close proximity to sciatic nerve. So is the v.l. ok to use?

i wouldn't inject thorazine im into the vl of a cachetic adult.

it requires a deep muscle.

but vl is fine for someone of normal or add'l weight.

20 years ago I got a shot of quinine in my left sciatic nerve - I can still feel it.

To avoid the sciatic nerve I was taught to use the lateral gluteal muscle. Place your index finger on the anterior iliac crest and your middle finger on the superior iliac crest (your right hand on the patient's left hip or your left hand on the patient's right hip). Inject inside the triangle formed by your two fingers and the ridge of the iliac crest - there is a lot of muscle under there and you'll never hit the sciatic nerve.
