Partial permanent disability

I have been on short term disability since Feb.3 and it runs out on 4/27. I know I qualify for partial permanent disability, but am unsure how to go about applying for it. Does my doc start the process or do I need to see a lawyer?

This is very distressing to me as, after 27 years, I will have to give up hands on nursing due to back problems. I had really found my niche in hospice and thought that's what I would retire doing.....not to be.

Any advice, support, prayers greatly appreciated.

At 'alternative nursing jobs' you stated "I will have to redirect my nursing career to something less physically stressful. Am considering nursing informatics, possibly legal nurse consulting."
