I am trying to find information on the PAMPER system for giving a pt report at end of shift or pt xfer.

I have searched the net but I mostly get stites related to diapers.

I did get a couple of hits for this site but only came up with info on SBAR, which, while nice enough is not what I need.

I think that for PAMPER, A= Allergies and that is all I know.

If you can be of help with this I would greatly appreciate it.

Or, even if you can point me at a website or suggest search terms to refine my google search.

Pregnancy / breastfeeding?

Ask all women between ages of 12 and 50:

Is there any chance you might be pregnant?

Have you used birth control (BC) consistently and correctly?

Allergies

Foods, chemicals, drugs, insect bites, cosmetics?

Medications

Current OTC, Rx, and BC pills, recreational drugs, alcohol

Previous chronic illness

Recurrent illness, multiple surgeries, family history

Emotional state

Psychiatric history, current reaction to illness

Recent injury, illness, ingestions

Recent accident or child/elder/spouse abuse/neglect

Exposure to communicable disease

Possible ingestion

Sorry, Acronym was the word I meant.

I'm looking for the PAMPER Acronym info to give report on pts leaving the ER & either being

transfered or admitted to the floors.
