hey, im a nursing student from the philippines, i need help in doing 2 ncp's for my cva pt. one should be a priority problem and the other is a risk diagnosis. i have three choices for my priority problem: ineffective cerebral tissue perfusion (, impaired physical mobility and selfcare deficit. I cant use the nsg dx risk for impaired swallowing (it's not applicable to my pt, she can eat without problems, she just needs assistance in eating)and unilateral neglect (i did not assessfor any sensory loss of visual field) please help me.. need to finish this by tomorrow.

You could always do Knowledge deficit: stroke related to lack of experience. And goals could be describe risk factors and signs and symptoms of stroke... just an idea from a neuro nurse ;) Hope it helps

This was just asked this week on this same forum and received a lot of attention and I posted a whole bunch of other previous threads that addressed care plans for stroke patients. Nursing diagnoses are based upon the symptoms your patient has.
