How would you tell your boss this?

Here is the situation. I work in a residential facility for kids with psych/behavioral disorders. They currently give all kinds of OTC's with no standing order, just a consent upon admission from parent/guardian. I brought this up with the director twice--- both times she assured me we were covered just with the consent. I know better.....

And another thing that bothers me.... the med "system".

The kids are on lots of psych meds, with lots of changes, naturally. They come on bubble pak cards, from an outside pharmacy. When a kid is d/c, or a med is changed, they keep that med, and we use it to start another kid on it if we get an order or a new admission. They just cross the name out and write the new kids name. There is a closet FULL of meds. So when something new is started, we go to the cabinet, get it out and put that card in the kids binder. Now I know this isnt kosher, but I need something to go to her with, since she is obviously clueless that this cant happen.

How would you approach this with her without sounding confrontational?

Where can I get proof of this to show her?

I have decided.... I am going in tomorrow, and bringing it up, telling her we need orders for these otc's, and we cant be administering meds the current way.I just havent figured out the exact words, and I am nervous about bringing it up..........

Keep in mind that she may try to bully you by threatening you with your job. Tell her that if she terminates you rather than fixes the problem, you will report the facility to the state. There are several agencies that would be interested in this, namely the state medical association because she is prescribing and dispensing meds without a physician's order.

Rnonoftoday,Ive tried to contact you on PM before. Ived worked for such a facility,for minute.We do not give out medications without standing DR orders!REmember your nursing license!Learn the laws regarding your license if you need refreshment .We have a Pediatrician and a PYSCHIaTRIST ON DUTY.wE HAVE STANDING ORDERS TO ADMINISTRATE OTC MEDICATIONS. When the Phsychiatrist changes orders,we write up a new order to the pharmacy,get the new meds from pharmacy, and put them on the med cart.The old orders medication is put in another med cart and we get rid of the medication with a licensed pharmacist at the end of each month.I believe it is against the law to give out the medications from the old orders.I love my job,but always remember your license comes first. You need to know your boundaries with you license.Also there has to be a policy manual to guide you.Did you see what the policy manual reads regarding these incidences??You could PM me If you want more info.We have a very reputable institution where I work. There are Three of us nurses.

Betcher bippy you should be concerned!!! By going along with this you are putting YOUR license on the line.

One way to go into this would be to ask her to show you where it says in writing that this is okay.

Also speak with the outside pharmacy that you use and ask them.And when we d/c a child we send the medication with them,and get a prescription fronm the physchiatrist for the next months worth of meds.Im not sure you boss is knowledgable with this job.

How do I get a standing order? How long are they good for? Who will give the standing order if we have no peds doc on staff? Believe me, she doesnt know these things, so I would like to go in with solutions. Is it really just a order the doc writes out, for everybody in residence? can you give me an example> I have actually never seen one...

I feel your frustration.At our facility we have a pediatrician who writes standing orders he reveiws them once a year and renews them .It is printed up and in our policy manual. Example" a child has a headache One 200 mg Motrin P.O. Q. 4 hours PRN"Or A child has a superficial abrasion." Cleanse with N/S apply bacitracin and DSD or temp above 100,Tylenol 250 mg. po q 4 hours.Or Diarreah,immodium dose q. 4 hours PRN after first diarrhea{example. Pm me
