concept map for left-side hemorrhagic stroke

I'm new to this site, so I hope I'm going about this the right way. I am a BSN student (second semester, junior year) and I need help with a concept map. I have never done one, usually we write the monsterous care plan, but I'm trying to do a concept map for extra credit in a class. My patient's medical diagnosis is left-side hemorrhagic stroke and I really have no idea how to get started. I would really appreciate some help. Thanks :nuke:

This is just a basic outline. Not sure how much detail you need.

Signs and symptoms - Tests - Diagnosis- Treatment -Rehabilitation

Signs and symptoms for a hemmorhagic stroke - worst headache of the pt life usually, possibly having dizzyness and may effect level of consciousness,speech, vision, strength, sensation

Test- neurological testing,CT of head, MRI of head, blood tests, urine drug screen ( ex.cocaine) , etc

Treatment (depends on how bad the stroke is) -blood pressure control with meds,swallow evaluation, stool hemoccult daily, surgical intervention if needed, etc

Rehabilitation - OT/PT in house, placement of pt in a rehab facility or nursing home etc
