While making my afternoon med pass I gave my DM/intractable pain patient his evening Novolin N and 4 mg Dilaudid, one hour later I find the guy pale and sweating. My first reaction was to give him some o.j. I just kept thinking
"cold and clammy...he needs some candy", but after two o.j.'s he still was not coming around. At this point his MD is in the room and several other nurses so we gave him Narcan and an amp of D50. After the Narcan and D50 he seems better but is mad as heck because he is now in pain. Once the room was cleared the patient's wife commented on how fast I reacted, I told her that something about him said "I need a little sugar" and the patient replies:
"YEAH AND YOU TOOK IT ALL WRONG"!

I don't get it.
As far as low blood sugar, nothing is better than refined sugar. Fruit sugar just doesn't work as fast or as well.
Was he low glucose or opioid OD?
