Okay so im freaking out, recently I administered a wrong formula to a pt and im freaking the heck out right now, pt was not harmed in the process. I wrote out an incident report and submitted it and I haven't heard back from management. Im really scared im gonna lose my license over this incident.

Unless the formula you used was one you concocted from antifreeze and strychnine you aren't going to lose your license over this. Breathe.

Take a breath, as wuzzie said, you're not going to lose your license. You made a mistake. And not even a really big mistake. It's unlikely that the TF you were supposed to hang was drastically different, unless your patient has a very specific sort of allergy, but you already said the patient wasn't harmed. All the brown bottles look pretty similar, could happen to anyone. But it will never happen to you again I'm sure.

It's considered a med error. It didn't harm the patient. You might get disciplined or coached. But you're not. going to lose your job, and you're certainly not going to lose your license.
