form 3 of ny application form for filipino nurses

Hi, Im currently in the process of applying for eligibility for the nclex exam for new york. I graduated and am licensed in the philippines. I was wondering about form 3. Is it needed in my application? And if so, should I send it to the PRC and how much payment do they require? Thanks!

Did you already submit and complete the cvs application with CGFNS? If the cvs is not complete no point in applying to NY for license by examination.

No need to submit the form 3. But apply first for CGFNS Credential Verification for New York State.
