Forgot to complete a bs check help!!

I can’t believe I forgot a 6am blood glucose check. The patient has orders for 6am readings on Monday, weds and Friday. He is not on insulin or has parameters. I looked in the glucometer and began to write down the last reading taken on the hole on the MAR and then realized that was not from the day I supposed to take it. I crossed it out and wrote error. Idk what to put as the reason for not doing it. The MAR is a mess now ?

It’s ok. Nothing bad is going to happen because you forgot a blood glucose check. You are human. You made a mistake. Find a way of better organizing yourself and move on.

No one is perfect. I remind myself that all the time. Also when my coworkers are feeling down I remind them as well.
