What would you do if you were working on the floor and at end of shift you realized that your patient had not received his 12pm dosage of Phenobarbital?

Tell the charge and oncoming nurses and write up a med error on myself.

This, and inform the ordering physician to find out if s/he would like the dose given off schedule, or skipped, or whatever. Even if it's the end of my shift, it would be my job to page the doc and wait for them to call back since it was my error. After hearing from the doc, I would discuss the whole issue with the patient.

if you were working 7-3 i would consider giving it, and calling doc and writing up the variance...if you worked 7-7 prob call doc and see what he/she wanted done, do that, then write up the variance...

I agree, I would of called the doctor to see if they wanted a dose given ASAP or wait for the next scheduled dose. Then I would write out an occurance report as a missed med and whatever the resolution was after discussing it with the doctor.
