Fluid Volume Deficient Care Plan

Can someone help me with writing my NANDA R/T AEB?

Here is what I had

Fluid volume deficient R/T effects of dehydration AEB diarrhea, syncope, lab diagnostics

but then I got to thinking....the R/T "effects of dehydration" is the same as fluid volume deficient, so isnt' this redundant?

Could I just say

Fluid volume deficient AEB ...

???

How about Fluid Volume Deficit:

Deficient Fluid volume r/t active fluid loss, failure of regulatory mechanisms.

Or for Dehydration:

Deficient Fluid volume r/t active fluid volume loss

Impaired oral mucous membranes r/t decreased salivation, fluid deficit

Ineffective health maintenance r/t deficient knowledge regarding treatment and prevention of dehydration

You can start by doing a spell check!

Ummm.. are you not using a care plan book? If you have a good care plan book, it pretty much lays everything right out there for ya. It will even list possible R/T's and AEBs (although it may be listed as something like "clinical manifestations"). Your outcomes/goals and interventions are also listed right there in your book.

If you're attempting to write care plans without a care plan book, you need to go buy one right now. Period.

Honestly, it's not really beneficial to you to get people to do your homework for you. Don't mean to make that sound rude. Many nursing students and even practicing nurses don't understand the importance of nursing care plans in school. Suffer through them and you'll be better for it in the end.

On the plus side, once you're a nurse, your care plans are all generated by a computer anyway.

Also, your AEB's are WAAAAY off. Deficient fluid volume as evidenced by amber-colored urine? Amber-colored urine could be considered a normal color. Amber may be a bit dark for normal urine, but it's not dark enough to be an AEB for deficient fluid volume. Respiratory distress as evidenced by a temp of 100.4? Tell me how an elevated temp is a sign of respiratory distress?

For your AEB, you're looking for something that definitively points to your nursing diagnosis. Deficient fluid volume as evidenced by decreased skin turgor. Ineffective breathing pattern as evidenced by use of accessory muscles for breathing. Respiratory Distress is actually not an approved NANDA nursing diagnosis.

After reading this, it's very clear that you're not using a care plan book. Do yourself a ginormous favor and get one now! Trust me, you'll feel much more confident when you're not just trying to make things up.
