Focus your care plan on things you as the Nurse can change, prevent, or otherwise intervene in.

You have a problem, what are you as the Nurse going to do about it?

use your assessment and history to formulate a plan.

Think about prevention, prevention of further complications from her diabetes and from her insulin, such as DKA, hypoglycemia etc. Look at the age of this patient, think about psychological and emotional feelings this patient may have. What things could you do to help keep the patient comfortable? If this is a new diagnosis then teaching will be huge, and even if it isnt new, teaching is always important. Not sure if you have a Care plan book, if not get one because it will help you throughout nursing school, and if you do have one use it because it is extremely helpful. I probably wouldnt have gotten through school without it. Good luck, I hope this helps!:wink2:

see this thread in the nursing assistant discussions forum on construction of care plans: https://allnurses.com/general-nursing-student/help-care-plans-286986.html - help with care plans. you need to start by doing an assessment of this patient and gathering all your abnormal data. problems are based upon evidence which consists of the abnormal data that exists. you will see me mentioning this all the time in my replies to questions about care plans. these previous care plan threads include diabetes as a problem as well as care plan construction information:
