First clinical assigment.........HELP!

I just received my very first clinical assignment(well 1st one all by myself) and this is it...........

30yr old male, pnuemonia, dehydration, IDDM, and hypertension.

I have a pretty good idea for a care plan but wanted opinions........

What would you be most concerned with? What would your interventions be? Just want a little encouragment , I guess to make sure I am on the right track.

Monitor for hypo and hyper glycemia...know signs. administer insulin. TCDB for pneumonia... Monitor saturations and if blood gases are done. Ambulate to help get static fluid out of lungs...

Also, encourage incentive spirometer, monitor I&O and VS, try to keep HOB at 30-45 degrees if tolerated. If you're giving meds know the BP prior to administration, you instructor with most likely ask. Good luck with your first assignment.

Also, being diabetic and just having had pneumonia myself, his blood sugars will be unstable because of illness and dehydration. His insulin needs will be different than when he is not ill.

pneumonia...I had one w/pneumonia last week, I did ineffective airway clearance r/t retained secretions and persistant cough.....for interventions, breathing and coughing exercises,etc.
