Filipino nurse dilemma in registration abroad

Hi guys... I'm in dilemma right now :confused: I hope somebody can help me on this. I just passed my board exam last July 3 & 4, 2010 and didn't take an oath because I went in US before the oath taking and registration was held... I got married last Sept. 24, 2010 (which is the reason why I came here :p) and eventually I started using my married name... I'm planning to take an oath in Philippine Embassy in Chicago by next month... My problem is which name should I use for taking an oath and special power of attorney, SINGLE NAME OR MARRIED NAME? If ever the consul adviced to file it on my married name what will be the consequences? Does my representative needs further documents for this to prove my marriage? Do PRC honor marriage certificate abroad? Please help... Thank you in advance...

Hi Shenjie.. sorry for the late reply, I just read your post now. I already took the oath last March in Chicago PI embassy. Check your local website state for PI embassy location. You should bring your board rating and passsing certificate from the PRC and a Special Power of Attorney for your reperesentative. I paid $25 for the authentication, $25 for the oath taking certificate, and $25 for the SPA. I still used my single name on authentications and certifications because the PRC do not accept marriage certificates from the US unless you will report the marriage in PI embassy that will take 2 mos. for them to file it in the NSO. PRC also preferred to file it personally when changing to married name. If you have anymore questions just pm me :)

Hi pinx23... I made my own SPA :) I can send to your email my template if you want just pm me your email address. You will get the certificate of board rating and passing in PRC. Send an authorization letter attached with the copy of your ID's to your representative. Always give your representative an authorization letter in every transaction you'll make in PRC. You will bring the original copy of certificate of board rating and passing to Chicago PI embassy together with your SPA. They will give you all the signed and sealed documents at the same day you'll have your oath taking. So wear a nice clothes for picture taking :lol2: Then send all those documents to your representative. I'll be glad to help you if you have anymore questions :)

Oh pinx23! I forgot to tell you to print the oath form from PRC website. Other requirements are also listed on that form. Here are the rest of requirements:

1. Duly accomplished Oath Form or Panunumpa ng Propesyonal

2. Current Community Tax Certificate (cedula)

3. Two pieces passport size colored picture in white background with complete name tag

(for Oath Form and Certificate of Registration)

4. One pc. 1”x1” colored picture in white background with complete name tag

(for Registry of Professionals)

5. Two sets of metered documentary stamps (for Oath Form and Certificate of Registration)

6. One (1) short brown envelope with name and profession

New registrants should come personally to the PRC Central Office or to the nearest

Regional Office to file his/her application for registration and to affix his/her signature on the

registry book.

In case registrant is located abroad, please find below the following requirements :

1. Take the professional’s oath before the Philippine Consul.

2. Special Power of Attorney notarized by the Consul.

3. Copy of passport page showing personal data.

4. Copy of passport page showing date of entry abroad.

Board Certificates shall be claimed at the Central Office or the Regional Office where

the application for the same has been filed. Representatives must present Special Power of

Attorney and appropriate identification papers

Tourist visa for where? if the US then really makes no difference because you can not at the moment change your status if you find a job due to retrogression

okay your post is somewhat confusing.

you have a tourist visa, which I assume a US tourist visa since you posted your query under the OP's question related to US PRC oath-taking.

When are you planning on leaving the Philippines? Because considering the processing time of PRC, the result might be out by February, which is just weeks away. I know 2 of my batchmates who did their oath-taking abroad (Philippine Consulate).

Re: retaking the NLE if you fail. First, be positive. Second, if you really have to retake it, then I couldn't see any reason why you shouldn't. You really need it not just for nursing registration abroad but also for employment. A lot of doors will open for you if pass it. Trust me, I know.
