This is centered around Chronic Kidney Disease. I though ESRD is Acute Renal Failure. I think it Matters to the patients receiving a transplant?

ESRD is not acute renal failure. It is the end stage of chronic renal failure.
I do not understand your sentence about receiving a transplant. Could you clarify?
